Welcome to the Long House Surgery - Thank you for taking the time to complete our short questionnaire.

All new patients are invited for a Registration Health Check. This is not compulsory, but is very useful for us and is hopefully also of benefit to you as a patient. It gives us an opportunity to take base-line health measurements such as blood pressure, height & weight. At the same time we can discuss matters relevant to your general health and advise you about anything you might alter in your lifestyle, which could reduce the risk of future health problems.

You may not be aware, but unfortunately it can take several months to obtain your previous medical records, and it is clearly important for us to know as much as possible about you right from the start. This can be extremely useful if we are called in an emergency before your records come through.

Following completion of our questionnaire you may have found further details of past vaccinations etc., if so, can you please "up-date" us with the information at your registration health check.

When you attend can you please bring a fresh specimen of urine with you. A container can be obtained from reception.

Even if you feel well, or have recently had a health check elsewhere, we would still very much like to see you. It does give you the opportunity to meet at least one member of our practice team.
